Fire gear: age confirmation and assumption of risk

By purchasing fire performance equipment from BreakStream Arts LLC I confirm that:

1. I am at least 18 years of age.
2. I understand that fire performance carries a risk of serious injury, including burns, and that the equipment is sold for use only by trained persons with appropriate safety measures, a safety person and a fire-suppression plan in place.
3. No fuel is supplied with the equipment, and I am solely responsible for the fuel I choose and how I use it.
4. I assume all risk of injury or damage arising from my use of the equipment and release BreakStream Arts LLC and its members from any liability for that use, to the fullest extent permitted by law.
5. I will comply with all laws and venue rules that apply to fire performance where I use the equipment.
